Aftercare Case Manager
We currently have an exciting opportunity for a full-time, non-exempt Aftercare Case Manager for our organization. The Aftercare Case Manager will work directly with discharging residents, developing treatment and discharge plans, and providing crisis intervention. S/he will maintain computerized case management files and be responsible for special projects as assigned. We are looking for an energetic and outgoing individual to be a part of our team. Job duties will include the following:
Responsibilities:
- Assess residents upon their release from the program.
- Assist aftercare clients in maintaining recovery and staying connected to NDI's program and resources, including job training, education, and money management. Work closely with individuals in caseload.
- Coordinate medical, legal, dental and/or psychological services as needed
- Work collaboratively with case managers and employment specialists
- Facilitate substance abuse treatment groups and/or specialty groups as assigned, including evenings as needed
- Keep client files current through the computerized case management system and maintain ongoing files (both computer and handwritten)
- Participate in conferences, workshops, special projects, staff meetings, and other events as assigned
- Facilitate aftercare and alumni activities including events, parties, outings and talent shows.
- Complete other duties as assigned
Job Requirements:
- Educational background can be diverse, however, a Bachelor's degree is preferred.
- Drug and alcohol certification is required, but can be attained during employment.
- Must be knowledgeable in substance abuse issues, therapeutic interventions, and treatment planning, particularly as relevant to Therapeutic Communities
- Familiarity with Alcoholic Anonymous or other 12 step programs
- Working knowledge of community resources as they relate to substance abuse
- Experience and knowledge in working with a multidisciplinary treatment team
- Word processing skills required
- Veterans preferred
- Three (3) years of work experience is preferred
- Minimum of one (1) year clean and sober, if in recovery
