Housing Case Manager
Classification: Non-Exempt
Reports to: Program Manager
Summary
Works directly with residents in developing treatment and discharge plans, providing crisis intervention, individual counseling and group therapy. Maintains computerized case management files and is responsible for special projects as assigned. Focuses on client housing needs and trains other staff in providing services.
Job Duties
- Help residents of New Directions obtain and maintain housing.
- Meet with landlords and housing managers as needed.
- Make home visits to graduates to help them maintain housing.
- Assess resident readiness for housing
- Develop individualized treatment plans for housing maintenance
- Work closely with assigned case manager to insure appropriateness of services and follow up as needed.
- Coordinate medical, legal, dental and/or psychological services upon obtaining housing
- Work consultatively with other staff to ensure appropriate levels of treatment
- Facilitate housing acquisition and maintenance support group as needed
- Keep client files current through the computerized care management system and maintain ongoing files both on computer and handwritten
- Participate in conferences, workshops, special projects, staff meetings, and other duties as assigned
- Facilitate, where appropriate, aftercare and alumni activities
- Attend trainings and stay aware of housing options. Train other staff in housing issues as needed.
- Oversee the Shelter Plus Care programs.
- Maintain a list of housing options for graduates, including Sober Living programs and Shelter Plus Care housing providers.
- Work closely with the Aftercare Case Manager.
- Other duties as assigned.
Job Requirements
- Knowledge of housing, Section 8 Programs, Shelter Plus Care and other housing program
- Ability to work with landlords and present residents in a positive light
- Knowledge of the 12 steps of Alcoholic Anonymous or other fellowships
- Working knowledge of community resources as they relate to substance abuse and housing retention
- Experience and knowledge in working with a multidisciplinary treatment team
- Word processing skills required
- Veterans preferred
- Three (3) years of work experience is preferred
- Minimum of one (1) year clean and sober, if in recovery.
Education: Educational background can be diverse; however, a Bachelor's Degree is preferred.
