CASE MANAGER
We are currently in search of a full-time, non-exempt Case Manager. The Case Manager works directly with residents in developing treatment and discharge plans, providing crisis intervention, individual counseling and group therapy. Maintains computerized case management files and is responsible for special projects as assigned.
Job Duties:
- Assess new residents during screening, intake and orientation
- Assess level and degree of substance abuse problem in those residents assigned (15-25). Aid in determining course of treatment and recovery
- Develop individualized treatment plans and monitors progress in achieving goals
- Assist residents in planning each phase of recovery, including job training, education, and money management.
- Provide ongoing support, crisis intervention and relapse prevention groups and seminars
- Coordinate medical, legal, dental and/or psychological services as needed
- Work consultatively with other staff to ensure appropriate levels of treatment
- Facilitate substance abuse treatment groups and/or specialty groups as assigned, including evenings as needed
- Oversee daily activities of the house
- Keep client files current through the computerized care management system and maintain ongoing files both on computer and handwritten
- Participate in conferences, workshops, special projects, staff meetings, and other duties as assigned
- Facilitate, where appropriate, aftercare and alumni activities
Job Requirements:
- Knowledgeable in substance abuse issues, therapeutic interventions, and treatment planning, particularly as relevant to Therapeutic Communities
- Knowledge of the 12 steps of Alcoholic Anonymous or other fellowships
- Working knowledge of community resources as they relate to substance abuse
- Experience and knowledge in working with a multidisciplinary treatment team
- Word processing skills required
- Veterans preferred
- Three (3) years of work experience is preferred
- Minimum of one (1) year clean and sober
- Educational background can be diverse; however, a Bachelor's Degree is preferred.
- Drug and alcohol certification is required under the following conditions: 1) The employee must be registered within six months of hire &
2) The employee needs to be certified within five years of his/her
registration date.
